Job Title: Social Worker (Entry Level to 2 Years of Experience)

Compensation: $21.50/hour
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (with some flexibility)

Job Summary
We are seeking a compassionate and motivated Social Worker to join our team. In this role, you will provide goal-setting, case management, crisis intervention, and supportive services to help residents achieve financial self-sufficiency and secure permanent housing. This position offers the opportunity to make a meaningful impact in a collaborative and supportive environment.
Key Responsibilities

Goal Setting & Case Management:
- Develop and monitor individualized program plans with participants within 30 days of move-in.
- Assist in removing barriers to achieving financial and housing goals.
- Plan and monitor next-step housing strategies.

Crisis Intervention & Support:
- Provide crisis intervention services as needed.
- Offer intensive outreach and support for participants in urgent or complex situations.

Participant Support & Advocacy:
- Advocate for participants with external agencies and connect them to community resources.
- Assist participants in accessing entitlements, such as DHS and SSA benefits.
- Educate participants on daily living skills, including nutrition and personal hygiene.

Coordination & Communication:
- Maintain strong coordination and communication between all sources of participant support.
- Attend community meetings and professional training opportunities to stay informed and network.

Reporting & Documentation:
- Prepare monthly reports highlighting caseload progress, accomplishments, and challenges.
- Complete participant intake procedures and eligibility determination.
- Conduct household inspections as part of individual program plans.

Collaboration & Follow-up:
- Work closely with participants’ educational programs, employers, and childcare providers.
- Identify and refer participants to appropriate resources to achieve program goals.
Qualifications
- At least a two year degree in Human Services + field experience OR
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
- Must have a valid driver’s license, reliable transportation and the willingness to make client visits.
- 0-2 years of experience in social work, case management, or a similar role.
- Strong communication, organizational, and problem-solving skills.
- Ability to advocate for participants and build relationships with community resources.
- Familiarity with housing programs, entitlement applications, and crisis intervention is a plus.
